Insurance Information
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.
I’d recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
A brief explanation of insurance coverage
An explanation of insurance co-pays, co-insurance & deductibles:
Health insurance can be confusing to understand. At your first visit, you will provide Lindsay with your health insurance card and she will submit claims to your insurance. Insurance companies can take a while to process claims. Once a claim has been processed, both you and Lindsay will be provided with an Explanation of Benefits (EOB) that explains what your plan paid Lindsay and what you owe Lindsay for your share of the care provided. Most plans have some level of deductible, co-pay or co-insurance.
A deductible is the amount you pay for health care services before your health insurance begins to pay. For example, let’s say your plan’s deductible is $5,000. That means for most services, you’ll pay 100 percent of your medical and pharmacy bills until the amount you pay reaches $5,000. After that, you share the cost with your plan by paying coinsurance and copays.
Coinsurance is your share of the costs of a health care service. It’s usually figured as a percentage of the amount your plan allows to be charged for services. You will start paying coinsurance after you’ve paid your plan’s deductible. For example, if you have a plan with a $5,000 deductible, once you’ve paid $5,000 out of pocket to meet that deductible, your plan will apply a percentage to your total bill. If your therapy visit costs $200 and your co-insurance is 50%, your plan will pay $100 and you will be responsible for the other 50% of the cost of the visit.
A copay is a fixed amount you pay for a health care service, usually when you receive the service. The amount can vary by the type of service. You will pay your co-pay and your insurance plan will cover the remaining cost of the visit. For example, your plan may require that you have a $30 copay for each visit.
It is also possible that your insurance doesn’t have a copay or a deductible. There are dozens of insurance plans available and if you have questions about your specific policy, it is best practice to contact your insurance company directly to determine where you are at for the year with your deductibles, co-pays and co-insurance. This amount is set by the insurance company and often starts over in either January or July each year. When your plan rolls over for the year, you will be responsible for meeting your deductible, co-pays and co-insurance for the new plan year.
Co-pays & Co-insurance Payment Policy
Please note that clients may be responsible for a co-pay or co-insurance for each of our sessions. Visionary Mental Health Services, LLC advises you to contact your health insurance provider prior to our first session to determine what your benefits are. Please note that you will be responsible for any portion of the fee for services that your insurance does not cover. Clients are encouraged to make their co-pays at every session or set up a payment plan to be billed monthly for their co-pays.
Payment
To reduce collection efforts, it is the policy of VMHS, LLC to obtain and maintain a valid debit/credit of Health Savings Account card on file for every client. Please review the Financial Policy form listed on the “Forms” page for more information. I accept cash, check, and all major credit cards as forms of payment. I also accept payment via PayPal and you can pay directly on my site on this page. VMHS, LLC also accepts HSA accounts as a way to pay for therapy. Clients who are utilizing the sliding fee scale are not required to maintain a valid credit/debit card on file for payment.
Cancellation Policy
A therapy appointment is a time that is specifically allotted just for you. Visionary Mental Health Services, LLC requests 24 hours’ notice of the cancellation of any appointment. If a client no-shows or cancels an appointment at the last minute, VMHS, LLC cannot fill that spot with another client. Due to an appointment being reserved for you and that it cannot be filled, should a last-minute cancellation or no show occur, you will be billed $75 for the missed appointment time.
Please note that this fee is NOT billable to insurance and that you will be responsible for the bill. VMHS, LLC understands that emergencies and illnesses do occur, and some exceptions may be made for these events.
Clients utilizing the sliding fee scale for services are not subject to the cancellation fee policy and will not be charged for no-shows or last-minute cancellations.

Sliding Fee Scale
It is the mission of Visionary Mental Health Services, LLC (VMHS, LLC) to provide high-quality behavioral health services to individuals, couples, and families, regardless of their ability to pay for those services. To make this feasible, VMHS, LLC offers a sliding fee scale to clients who do not have health insurance or other means to pay for services. It is the policy of VMHS, LLC to not discriminate against clients based on race, religion, gender, identified gender, sexual orientation, socioeconomic status or ability to pay for services. VMHS, LLC strongly believes in being an inclusive and welcoming environment for all clients.
Clients that are interested in accessing services under the sliding fee scale are encouraged to print the documents listed on this page, complete the application, provide the necessary proof of income documents and submit the application for review. Documents can be scanned and emailed over to VMHS, LLC by using the email address of Lindsay@visionarymentalhealth.com , can be faxed to 605-385-0045, or can be mailed or hand-delivered to 1709 N. Lincoln Ave Suite #102 in Pierre, SD. Clients will be notified within 5 days of completion of the application and submission of the required forms if they are approved for sliding fee scale services and will be scheduled for services immediately after their approval. Clients that are denied for services under the sliding fee scale will also be made aware of this within 5 days.
